Activation of TAL1 characterizes up to 60% of cases of human T cell acute lymphoblastic leukemia, making it the most frequent gain-of-function mutation observed in this disorder. TAL1 (also designated SCL) is a serine phosphoprotein and basic helix-loop-helix transcription factor known to regulate embryonic hematopoiesis. This transcription factor binds as a heterodimer with E2A and HEB/HTF4 to a nucleotide sequence motif termed the E-box. In addition, leukemogenesis is accelerated dramatically by transgenic co-expression of TAL1 and the catalytic subunit of casein kinase IIHLH transcription factors. Cathepsin K is a lysosomal and secreted cysteine proteinase involved in bone remodeling and resorption. This protein, which is a member of the peptidase C1 protein family, is predominantly expressed in osteoclasts. However, the encoded protein is also expressed in a significant fraction of human breast cancers, where it could contribute to tumor invasiveness. Mutations in this gene are the cause of pycnodysostosis, an autosomal recessive disease characterized by osteosclerosis and short stature. Drebrins (developmentally regulated brain proteins) are cytoplasmic proteins that bind F-actin in the brain and are involved in cell migration, extension of neuronal processes and plasticity of dendrites. There are three isoforms: two embryonic types (E1 and E2); and an adult type (A), generated by alternative RNA splicing from a single Drebrin gene. Drebrins are expressed mainly in brain neurons but are also found in skeletal muscle, heart, placenta, pancreas and kidney. Drebrin has been designated as a marker of the dendritic spine. Decreases in Drebrin levels in the brain have been been associated with Alzheimer's disease and Down syndrome. DC-SIGN is a transmembrane receptor that is expressed on the surface of dendritic cells and macrophages. It is involved in the innate immune system and recognizes numerous evolutionarily divergent pathogens ranging from parasites to viruses. The protein is organized into three distinct domains: an N-terminal transmembrane domain, a tandem-repeat neck domain and C-type lectin carbohydrate recognition domain. The extracellular region consisting of the C-type lectin and neck domains has a dual function as a pathogen recognition receptor and a cell adhesion receptor by binding carbohydrate ligands on the surface of microbes and endogenous cells. The neck region is important for homo-oligomerization, which allows the receptor to bind multivalent ligands with high avidity. Alpha-fetoprotein (AFP) is an oncofetal glycoprotein with a single chain of 70 kDa. This MAb is highly specific to AFP and shows no cross-reaction with other oncofetal antigens or serum albumin. AFP is normally synthesized in the liver, intestinal tract, and yolk sac of the fetus. Recognizes a protein of 130 kDa, identified as Desmoglein-3 (DSG3). This MAb is highly specific to Desmoglein-3 and does not cross-react with other members of the Desmoglein-family. DSG3 is a calcium-binding transmembrane glycoprotein component of desmosomes in vertebrate epithelial cells. Research has shown that DSG3 has a very high sensitivity (80%) and specificity (100%) in recognizing squamous cell carcinoma (SqCC). Therefore, DSG3 is considered a very important marker for lung SqCC and can be a useful ancillary marker to separate SqCC from other subtypes of lung cancer. The protein encoded by this gene is a homeobox-containing developmental regulator associated with liver development. The encoded protein binds to the alpha-fetoprotein (AFP) gene promoter and increases the expression of AFP. This gene is overexpressed in some lung cancers and is linked to poor patient survival, possibly due to its resistance to cisplatin. This gene is aberrantly methylated in pancreatic cancer, deleted in squamous cell lung carcinomas, and acts as a tumor suppressor in esophageal cancer. Recognizes an oncofetal antigen of 220 kDa, identified as a tumor-associated glycoprotein (TAG-72) with properties of a mucin. This MAb defines the mucin-carried sialylated-Tn epitope. TAG-72 is usually expressed by adenocarcinomas, but is negative in mesotheliomas. Studies have reported that this antibody has 80% sensitivity and 93% specificity for pulmonary adenocarcinoma. Therefore, TAG-72 is a useful marker to distinguish between mesothelioma and adenocarcinoma. However, false positive reactions can occur so results must be interpreted with the utmost caution. This antibody may be useful in the differentiation of non-small cell carcinomas from small cell carcinomas of the lung. The combined use of anti-TAG-72 and anti-GCDFP-15 is valuable in the diagnosis of apocrine carcinoma. Calretinin (also known as Calbindin 2) is a member of the troponin C superfamily. Proteins in this family have 6 EF-hand domains which bind to calcium. Calretinin is a vitamin D-dependent calcium-binding protein involved in calcium signaling. It is present in subsets of neurons throughout the brain and spinal chord, including sensory ganglia. Antibodies to calretinin are useful in differentiating mesothelioma from adenocarcinomas of the lung. They can also aid in differentiating adrenal cortical neoplasms from pheochromocytomas.
